- ✪ ✪ ✪ ✪ ✪Before you make an appt at this office you'd better ask what the damage will be. It's unlikely they follow the Dental Assoc Fee Guide as my two other root canals done by Dr Qualley and Dr Levin were completely covered by my Pacific Blue Cross extended plan - here they charge $349.40 ABOVE the plan coverage for one "3 root" root canal". The worst part - they didn't tell me up front, just surprised me with the bill after the work was done and sent a little extra bill in the mail.by A Google UserDecember 27, 2016
